1 TOTAL ANNUAL WAGE HOURS DO NOT count more than 2080 hours for any employee. Hours attributable to a sole proprietor, a partner in a partnership, a shareholder owning more than 2% of an S corporation and any owner of more than 5% of other business should NOT be included.
2 TOTAL ANNUAL WAGES PAID Total wages paid during the tax year as defined for FICA purposes (without regard to the wage base limitation)
3 TOTAL ANNUAL ELIGIBLE PREMIUMS Enter the amount your company contributed toward medical premium. EXCLUDE those amounts attributable to a sole proprietor, a partner in a partnership, a shareholder owning more than 2% of an S corporation and any owner of more than 5% of other business. You MUST contribute AT LEAST 50% toward the employee only premium.
